• Diagnostic x-rays every 12 months
• 1 emergency exam per year if needed

Perio Membership Plan
You save on everything from cleanings and fillings to cosmetic procedures!

For patients who need periodontal maintenance due to prior diagnosis and treatment of periodontal disease.

• Perio maintenance cleanings (3-4 per year)
• 1-2 dental exams,
• needed x-rays
• 1 emergency visit per year if needed

Kids Membership Plan

Program Guidelines
• Patients portion of bill is due the day of service
• Cannot be used in conjunction with another dental plan or dental insurance
• NON-REFUNDABLE
